The introduction of the electronic health card is the biggest IT project in the German health care sector to date. However, the project was characterized most by the protracted introduction phase, changing project organization, and diverging interests between the key players.

This study begins with a comparative account of the developments in the field of e-health within Europe and foregrounds the current German developments by reference to the electronic health card with its additional applications. Building on this, the study then focuses on project management and user acceptance for the introduction of the electronic health card, based on a written survey taken among project participants and on existing research findings. The causes for the negative evaluations of the project management are thereby found to lie in the business model of the responsible project organization and the divergence of interests among the project stakeholders. Insufficient user acceptance among healthcare providers in the tests conducted can be explained by poor practicability for everyday work operations. Beyond these practical considerations, the introduction of the electronic health card has had a major impact on the entire healthcare sector.

The technical change in the German healthcare sector resulting from the introduction is characterized by the high degree of invasiveness of new technologies and the varying receptivity among affected users. Healthcare providers have been more strongly affected by the introduction than cost bearers and consistently oppose certain technological applications in the context of the electronic health card.
