The annually published excerpts from the Federal Physicians' Register of the National Association of Statutory Health Insurance Physicians (Kassenärztliche Bundesvereinigung) show a shift in the composition of health care providers participating in SHI-accredited medical care: While the share of employed physicians in the outpatient sector is increasing significantly (especially since 2004 due to employing medical care centers) and in 2020 already more than every fourth physician belonged to this group, the number of physicians in private practice is decreasing. In this context, this thesis aimed to analyze the situation and individually present premises in which a physician explicitly chooses an outpatient mode of professional practice - given the rising employment numbers, the focus was on the choice of outpatient employment, with the branch office serving as a contrast. Against this background, a mixed-methods approach was chosen: In the first step, an explorative qualitative survey was conducted to derive a theory of outpatient physician practice behavior. Based on the delineation of different types of physicians for employment and establishment, relevant situational factors influencing the choice of practice type emerged, which were subsequently tested by a quantitative standardized survey in the second step.
From the perspective of action theory, outpatient employment can be defined overall as a temporary occupational-bio-graphic phenomenon for the purpose of risk management. In contrast, settlement can be described as the realization of opportunities. On the one hand, this results in possibilities for the classification of target groups for the individual types of medical practice. On the other hand, there are practical implications for targeted influence by practicing physicians, employing institutions and intermediaries in the outpatient sector.
